[CAP-PAO] SUI D-6 form

Yeah, I found that.  But if you click on the D-6 PA worksheet (PDF file
along the right side of the page), it only gives you the first and second
questions (3 pages only).

Is that it for squadron SUI questions for PAO -- only two questions?
Granted, they removed the Crisis Comm plan for squadrons, but I seem to
remember a few more questions.
