[CAP-PAO] SUI D-6 form

[D Miller]

Honestly, I haven't been involved in squadron SUI preparation for several
years -- being mainly involved in the Wing CI, SAV, OpsEval process.

Can you forward the SUI D-6 section questions so it can jog my memory?
